Medical Forms & Required Shots

  1. Do I really need to visit the doctor?
  2. Must the Medical Release form be notarized?
  3. What shots do I need?
  4. Do we have to have medical insurance to go on projects?

1.) Do I really need to visit the doctor?
Anyone under the age of 18 needs to have a doctor's signature on the Medical Release form. If the student has had a physical within the last year, they can usually go back to the same doctor and just have him sign the form. For those over 18, unless they have been under a doctor's care within the last year, they do not have to have a doctor's signature, but do need to sign the form and return it to AIM.

2.) Must the Medical Release form be notarized?
Yes.

3.) What shots do I need?
ALL projects require an updated Tetanus shot.
Hepatitis A is recommended but not required for the following projects:
  • New Orleans
  • Nuevo Laredo
  • Ecuador - Typhoid recommended but not required
  • Iquitos, Peru - Typhoid, Malaria strongly recommended but not required
  • Panama
  • Lima, Peru

4.) Do we have to have medical insurance to go on projects?
We require that each person going on the mission project sign an AIM medical release form and send a copy of it to the AIM office while your group leader keeps the original with him or her during the project. We strongly encourage medical insurance but do not require it; however, each person will be responsible for any medical cost they incur on the project.
